Nutrient Depletion While on Metformin

Metformin, also known as Glucophage or Glumetza, is often first-line therapy in people with non-insulin-dependent Type 2 Diabetes. It lowers blood sugar by decreasing glucose production by the liver, decreasing absorption of intestinal glucose, and increasing muscle and fat cell sensitivity to insulin. These multiple mechanisms of action are the reason Metformin is usually first-line therapy for diabetic patients. While Metformin is decreasing blood glucose in the body, it is simultaneously depleting nutrients that are crucial for proper bodily functioning, an unfortunate side-effect. Vitamin B12 and folic acid are just 2 of the essential nutrients that Metformin can deplete.

Vitamin B12 is a water-soluble vitamin that is essential for nerve conduction, mental functioning, red blood cell formation, and DNA synthesis. A deficiency in this nutrient can affect mood and energy. It can also cause serious and sometimes permanent neurological damage as well as anemia. Symptoms of low vitamin B12 include neuropathy, certain types of anemia, confusion, chronic fatigue, memory loss, mood changes, and eventually even dementia. This is why it is so important that these levels are monitored for all patients on Metformin.

Helping with Low B12 Levels

Fortunately, there is an easy solution to prevent lower vitamin B12 levels while on Metformin. Eating foods rich in vitamin B12 (beef, liver, chicken, salmon, tuna, clams, eggs, yogurt, cheese) or taking a supplement can help maintain B12 levels. The recommended daily intake for adults is 2.4 micrograms. This daily dose can change depending on age, medical conditions, pregnancy, and breastfeeding.
